Care Management is a private pay service. Fees are not generally covered by Medicare, Medicaid, or most health insurance policies. Occasionally fees are covered under a Long-Term Care Insurance Policy, and we will be happy to assist you in determining if your LTC plan would reimburse you for all or part of our services.
The total cost of Care Management services will depend on the frequency and
intensity of intervention that is requested by the 
client. Professional Care Management involves and in-depth assessment, the development of an individualized plan of care, service arrangement with on-going follow-up and monitoring. While we believe this is the most effective way to serve our clients' needs and enhance their quality of life, we also realize that budget restraints may not permit them to take advantage of the full service we provide.
In addition to our standard care management services, we also provide professional consultations, by phone or in person, at an hourly rate. This in no way replaces the benefits that can be derived from full Professional Care Management services, but it will permit those on a limited budget the opportunity to learn about various options, services, and community resources.
We strongly believe that information is power and that the valuable information we will provide during a one-hour telephone consultation will permit you to make better choices and more informed decisions regarding specific problems or concerns.
As Care Managers we believe our services will save you time, money, and reduce family burden through coordination of appropriate care and resources and avoidance of unnecessary expenditures. One of our many goals is avoidance of crisis, unnecessary hospitalizations, and inappropriate placements.
In addition to accepting payment by cash or personal check, we are pleased to announce we are able to accept payment via VISA, MasterCard, and Discover card.
